{"id":17364,"date":"2017-12-04T09:27:01","date_gmt":"2017-12-04T08:27:01","guid":{"rendered":"https:\/\/www.zerodonto.com\/en\/?p=17364"},"modified":"2017-12-04T17:42:37","modified_gmt":"2017-12-04T16:42:37","slug":"root-canal-shaping-expansion-endoshaper-endofinisher","status":"publish","type":"post","link":"https:\/\/www.zerodonto.com\/en\/2017\/12\/root-canal-shaping-expansion-endoshaper-endofinisher\/","title":{"rendered":"Root canal shaping with latest generation expanding instruments"},"content":{"rendered":"<h2 class=\"p1\"><span class=\"s1\">INTRODUCTION<\/span><\/h2>\n<p class=\"p1\"><span class=\"s1\">The need to shape a root canal<span class=\"Apple-converted-space\">\u00a0 <\/span>root canal thoroughly is surely the primary purpose of any proper endodontic treatment. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Only by removing the canal content (whether organic or not) irrigants are able to carry out their action,<span class=\"Apple-converted-space\">\u00a0 <\/span>allowing the sealer <span class=\"Apple-converted-space\">\u00a0 <\/span>and the filling materials to seal the endodont.<\/span><!--more--><\/p>\n<p class=\"p1\"><span class=\"s1\">For this purpose, different instrumentation methods have been developed over the years, making<span class=\"Apple-converted-space\">\u00a0 <\/span>use of higher and higher performance<span class=\"Apple-converted-space\">\u00a0 <\/span>materials; however,<span class=\"Apple-converted-space\">\u00a0 <\/span>studies<span class=\"Apple-converted-space\">\u00a0 <\/span>carried out so far,<span class=\"Apple-converted-space\">\u00a0 <\/span>by using Micro-Ct, have shown that an endodontic area, up to 70%, depending on the root canal anatomy<span class=\"Apple-converted-space\">\u00a0 <\/span>and the root canal section examined,<span class=\"Apple-converted-space\">\u00a0 <\/span>is left untreated, i.e. uncleaned. However, there are still doubts about their efficacy, especially if we take into account the cost-benefit ratio.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">As for As for new sealers (bioceramics), please refer to a further article that will deal with their characteristics and instructions for use.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">In this article, we are going<span class=\"Apple-converted-space\">\u00a0 <\/span>to describe the innovation brought to product research, regarding Ni-Ti instruments.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Dealing with instruments and instrumentation, the most worrying issue is the risk of fracture to which they are subject. Unless we are dealing with native structural imperfections (creep fracture) [23], first of all,<span class=\"Apple-converted-space\">\u00a0 <\/span>we have to take into account the accumulation of cyclic fatigue to which the instruments are subject [24], and the torsional stress produced during cutting maneuvers.<\/span><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/pXa7Fob6OUc?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p class=\"p1\"><span class=\"s1\">Of course, the shaping quality<span class=\"Apple-converted-space\">\u00a0 <\/span>goes hand in hand with the cleaning quality, because as long as<span class=\"Apple-converted-space\">\u00a0 <\/span>cleaning is carried out ,<span class=\"Apple-converted-space\">\u00a0 <\/span>irrigants<span class=\"Apple-converted-space\">\u00a0 <\/span>can penetrate and clean canal walls.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">In order to reduce the stress to which the instruments<span class=\"Apple-converted-space\">\u00a0 <\/span>are subjecsubject there was an initial attempt to modify the traditional continuous rotation movement by introducing reciprocating movementsreciprocating movements [25].<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Considering<span class=\"Apple-converted-space\">\u00a0 <\/span>that some studies claim that the reciprocation <span class=\"Apple-converted-space\">\u00a0 <\/span>lengthens the life of instruments [26,30], some companies produced motors with specific movements, which increasingly reduce<span class=\"Apple-converted-space\">\u00a0 <\/span>stress<span class=\"Apple-converted-space\">\u00a0 <\/span>against canal wall, by creating dedicated movements<span class=\"Apple-converted-space\">\u00a0 <\/span>as in the systematic Adaptive Motion Technology \u2122 (Kerr) [27] or Optimum Torque Reverse \u2122 (Morita) [31] (Fig.3-4).<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17369\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-16.jpg\" alt=\"\" width=\"900\" height=\"518\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-16.jpg 900w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-16-300x173.jpg 300w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-16-768x442.jpg 768w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-16-695x400.jpg 695w\" sizes=\"auto, (max-width: 900px) 100vw, 900px\" \/><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17370\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23.jpg\" alt=\"\" width=\"220\" height=\"218\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23.jpg 220w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23-150x150.jpg 150w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23-60x60.jpg 60w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23-32x32.jpg 32w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23-50x50.jpg 50w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23-64x64.jpg 64w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23-96x96.jpg 96w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-23-128x128.jpg 128w\" sizes=\"auto, (max-width: 220px) 100vw, 220px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">The aim is to reduce the surface contact of the instrument with the canal walls by preventing<span class=\"Apple-converted-space\">\u00a0 <\/span>the blades from reaching<span class=\"Apple-converted-space\">\u00a0 <\/span>stress point which may<span class=\"Apple-converted-space\">\u00a0 <\/span>cause their deformation and breakage.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">This happens mainly because all the instruments we are dealing with, work with<span class=\"Apple-converted-space\">\u00a0 <\/span>higher or lower torque, depending on the instrument design.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">However, these devices<span class=\"Apple-converted-space\">\u00a0 <\/span>have not led to the resolution of the above mentioned problems.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Therefore, an attempt was made to improve the efficacy of the instruments, by processing<span class=\"Apple-converted-space\">\u00a0 <\/span>Ni-TI bars with altered thermal cycles. This allowed<span class=\"Apple-converted-space\">\u00a0 <\/span>various companies manufacture instruments ,<span class=\"Apple-converted-space\">\u00a0 <\/span>with an Martensitic-Austenitic transition (Fig5) occurring at temperatures<span class=\"Apple-converted-space\">\u00a0 <\/span>which are different from the past<span class=\"Apple-converted-space\">\u00a0 <\/span>[32]; they are more flexible and have a definitely higher tolerance of cyclic and torsional fatigue(than the past) [33,34], even if at the expense<span class=\"Apple-converted-space\">\u00a0 <\/span>of cutting capacity.<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17371\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-17.jpg\" alt=\"\" width=\"600\" height=\"328\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-17.jpg 600w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-17-300x164.jpg 300w\" sizes=\"auto, (max-width: 600px) 100vw, 600px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">This led to the manufacturing of M-Wire \u2122 (Dentsply-Tulsa DentalSpecialities &amp; WDV), R-Phase \u2122 (Kerr), CM-Wire \u2122 (Colt\u00e8neWhaledent), Blue \u2122 (Dentsply-Tulsa DentalSpecialities), Gold \u2122 (Dentsply-Tulsa DentalSpecialities) and Max-Wire \u2122 (FKG) : they were<span class=\"Apple-converted-space\">\u00a0 <\/span>characterized by a martensitic\u2014austenitic phase transformation point at different temperatures, thus obtaining instruments able to work in the martensitic phase at room temperature and return to the austenitic if heated and brought to their phase transformation point.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Transition temperatures range from M-&gt; A are 17 \u00b0 C for R-Phase, 35 \u00b0 C for Max-Wire, 38 \u00b0 C for Blue, 50 \u00b0 C for M-Wire, 55 \u00b0 for CM and the Gold [23]<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">This way, the Ni-Ti polycrystalline structure<span class=\"Apple-converted-space\">\u00a0 <\/span>is found at room temperatures in more plastic phases which, depending on the various commercial specifications, give <span class=\"Apple-converted-space\">\u00a0 <\/span>individual instruments their characteristics.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">From a clinical point of view, this turns into the opportunity to use files with a predominant Martensitic phase in curved channels and to choose instruments with a predominant Austenitic phase<span class=\"Apple-converted-space\">\u00a0 <\/span>in all cases requiring greater rigidity of the instrument itself (calcified canals and retreatment).<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The use of these new alloys makes it possible to increase the security level while using the instruments , and to reduce the apex transport likely to occurr; [39] However, there are still problems.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Studies carried out on<span class=\"Apple-converted-space\">\u00a0 <\/span>reciprocal instruments, have reported<span class=\"Apple-converted-space\">\u00a0 <\/span>a high percentage of debris accumulation [28] which, in some situations (isthmuses and ramifications) occlude part of the canal (Fig6)<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17372\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-19.jpg\" alt=\"\" width=\"575\" height=\"151\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-19.jpg 575w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-19-300x79.jpg 300w\" sizes=\"auto, (max-width: 575px) 100vw, 575px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">The possibility to create dentinal micro-cracks during treatment at high torque has been reduced but not eliminated , [29,38] (Fig7), moreover, in some cases, the excessive flexibility of some instruments goes to the detriment of their cutting efficiency, but above all, an improvement of percentage of cleaned canal surface, mainly in oval canals, has not been achieved yet. instruments, wich are able to adapt themselves<span class=\"Apple-converted-space\">\u00a0 <\/span>to the canal anatomy<span class=\"Apple-converted-space\">\u00a0 <\/span>without causing any alteration,<span class=\"Apple-converted-space\">\u00a0 \u00a0 <\/span>but trying to reach as large a surface as possible, due to their morphological characteristics.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">This category includes SAF \u2122 (ReDent Nova-Israel) and Xp-EndoFinisher \u2122 and Xp-EndoShaper \u2122 (FKG- Swizerland) [43] and\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">\u201cGentleFIle System\u201d (MedicNRG,Kibbutz Afikim, Israel).<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2 class=\"p1\"><span class=\"s1\">XP-EndoFinisher &amp; XP-EndoShaper<\/span><\/h2>\n<p class=\"p1\"><span class=\"s1\">As early as 2010, ReDent-Nova launched on the international market the <a href=\"http:\/\/www.zerodonto.com\/2015\/06\/la-tecnica-self-adjusting-file\/\" target=\"_blank\" rel=\"noopener\">SAF<\/a> \u2122,<span class=\"Apple-converted-space\">\u00a0 <\/span>[1], which revolutionized the concept of cleaning and shaping, by manufacturing an instrument that broadens the contact area within<span class=\"Apple-converted-space\">\u00a0 <\/span>canal walls, thus<span class=\"Apple-converted-space\">\u00a0 <\/span>cleaning up to 70%-80 % of the endodont.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">In 2015, FKG manufactured an equally singular file, the XP-Endo-Finisher \u2122 (XPEF), which was born with the purpose of refining the canal after the shaping phase, in order<span class=\"Apple-converted-space\">\u00a0 <\/span>to increase the effect of irrigants and improve smear-layer removal<span class=\"Apple-converted-space\">\u00a0 <\/span>from the walls (fig1). <\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17377\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-15.jpg\" alt=\"\" width=\"715\" height=\"357\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-15.jpg 715w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-15-300x150.jpg 300w\" sizes=\"auto, (max-width: 715px) 100vw, 715px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">Unlike the SAF @,it can be used without a dedicated motor, and it works as a finisher at the end of an ordinary canal<span class=\"Apple-converted-space\">\u00a0 <\/span>instrumentation sequence.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The instrument, made of an alloy patented by FKG<span class=\"Apple-converted-space\">\u00a0 <\/span>called Max-Wire \u2122, and it remains<span class=\"Apple-converted-space\">\u00a0 <\/span>in the martensitic phase state at room temperature (fig2), then it reaches a phase transformation at above 35 \u00b0 C\u00a0<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17378\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig5b-72.jpg\" alt=\"\" width=\"800\" height=\"318\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig5b-72.jpg 800w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig5b-72-300x119.jpg 300w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig5b-72-768x305.jpg 768w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig5b-72-750x298.jpg 750w\" sizes=\"auto, (max-width: 800px) 100vw, 800px\" \/><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/dsYjSlIrc54?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p class=\"p1\"><span class=\"s1\">The instrument comes in a rigid plexiglass box, at the ends of which there are millimeter notches which make it possible<span class=\"Apple-converted-space\">\u00a0 <\/span>to calculate the working length (LDL) before inserting the file inside the canal.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The instrument diameter is ISO # .25 and its taper is equal to 0. It is used at speeds of 800-1000 rpm at a torque of 1 as it does not perform any active cutting action.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">In its austenitic phase, the file is characterized by the presence, in the last 10 mm of the working part, of a sort of hook shaped curve which allows to penetrate a larger canal surface than traditional instruments (fig. 3)<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17379\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-24.jpg\" alt=\"\" width=\"1015\" height=\"507\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-24.jpg 1015w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-24-300x150.jpg 300w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-24-768x384.jpg 768w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-24-750x375.jpg 750w\" sizes=\"auto, (max-width: 1015px) 100vw, 1015px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">As it is an extremely slim and flexible file, due to the centrifugal force, it generates a movement that gives continually<span class=\"Apple-converted-space\">\u00a0 <\/span>the working part<span class=\"Apple-converted-space\">\u00a0 <\/span>different trajectories that allow it to come into contact with 80% of the canal surface within a minute of work [20]. (Fig4.)<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17380\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-27.jpg\" alt=\"\" width=\"1280\" height=\"1062\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-27.jpg 1280w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-27-300x249.jpg 300w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-27-768x637.jpg 768w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-27-1024x850.jpg 1024w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-27-482x400.jpg 482w\" sizes=\"auto, (max-width: 1280px) 100vw, 1280px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">Several studies compared the XPEF cleansing capabilities to PUI \u2122, Vib-RingeSyringe \u2122 and PIPS, both in the phases of removal of dressing pastes (calcium hydroxide and antibiotics) [2,6,10, 11,12,13,14] and in those of removal of biofilm<span class=\"Apple-converted-space\">\u00a0 <\/span>grown in the canals [1,8,9,15,16]<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Considering not only the hypochlorite activation action<span class=\"Apple-converted-space\">\u00a0 <\/span>but also the action of contact with the canal walls, the XPEF has proven to be the most effective in almost all situations (video2). Only in some cases it has<span class=\"Apple-converted-space\">\u00a0 <\/span>proven to have a detergent capacity comparable to PUI, although in some work [2] it was not specified whether the irrigation solution was maintained above 35 \u00b0 C, a prerequisite<span class=\"Apple-converted-space\">\u00a0 <\/span>to getting maximum performance from the instrument.<\/span><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/ZHrjJtIEkzk?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p class=\"p1\"><span class=\"s1\">The comparative analysis between XPEF and the latest generation Ni-Ti instruments on the ability to disrupt biofilm confirmed that the latter still left a portion of unremoved bacteria in statistically significant values \u200b\u200b[1,9].<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Further analysis was carried out on the amount of debris ejected from the apex during processing and it was estimated that the only working action of the XPEF does not produce significant debris [3] (comparable to the amount of pulp produced by the SAF) but it is the pre-operational operating sequence that determines a greater production.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">A study by the Versiani group has shown that XPEF has better removed post-instrument debris in the lower molar roots, improving also the results obtained with the SAF \u2122 considered the gold standard for this purpose. [15]<\/span><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/dOrdTyJx514?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p class=\"p1\"><span class=\"s1\">Similar conclusions<span class=\"Apple-converted-space\">\u00a0 <\/span>were drawn in this study by PUI, but the latter method only works<span class=\"Apple-converted-space\">\u00a0 <\/span>to increase the action of the irrigants, but not to disaggregate the biofilm<span class=\"Apple-converted-space\">\u00a0 <\/span>on the walls. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">A study by Azim et al [5] evaluated the hypochlorite extrusion that could be caused by<span class=\"Apple-converted-space\">\u00a0 <\/span>using<span class=\"Apple-converted-space\">\u00a0 <\/span>XPEF, considering its swirling movement within the endodont, and it has emerged that comparing to Endovac@ the discharge of hypochlorite is bigger,<span class=\"Apple-converted-space\">\u00a0 <\/span>but comparable to that of other systems in use such as Endoactivator @.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Clinically, it was also attempted to evaluate the possibility of removing gutta-percha from the canals using the XPEF at the end of ordinary<span class=\"Apple-converted-space\">\u00a0 <\/span>sequences and, as already shown for the removal of dressing pastes, the file proved to be very effective [4,17] with statistically significant values,<span class=\"Apple-converted-space\">\u00a0 <\/span>but still dependent on the maneuvers previously performed.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Unfortunately, the need to use preparatory methods before using<span class=\"Apple-converted-space\">\u00a0 <\/span>XPEF did not solve the problem related to, for example, the formation of dentin crack, or the massive production of debris; as in some studies [3,17] the final quality of the result was often affected by the initial preparation performed.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">To solve some of these problems, and to better define the concept of three-dimensional canal shaping, in 2016, FKG introduced on the market an instrument for shaping the entire endodontic: XP-Endo-Shaper \u2122 (XPES).<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The instrument is still manufactured in Max-Wire \u2122 alloy, but has a 4% taper and an ISO # .30 apex. The anatomy of the instrument is unusual; in fact it is not the classic conical instrument that impresses its shape in the canal, but it has waves throughout its body which, during the expansive rotation phase, make the instrument impact the canal surface in the best way (Adaptive Core Tecnology).<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The tip also has peculiarities (Booster-Tip). It has six cutting blades for optimum file guidance in the canal; the minimum diameter of the tip corresponds to an ISO # .15 reaching the maximum diameter of # .30. If used at body temperature, the instrument returns to the shape outlined for the austenitic phase; the rotation to which it is subjected (800 rpm), and its excellent flexibility make it possible to cover taper up to 8% and canalar diameters up to# .90. (fig5 -6)<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17381\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig6-75.jpg\" alt=\"\" width=\"800\" height=\"450\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig6-75.jpg 800w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig6-75-300x169.jpg 300w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig6-75-768x432.jpg 768w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Fig6-75-711x400.jpg 711w\" sizes=\"auto, (max-width: 800px) 100vw, 800px\" \/> <img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17382\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-42.jpg\" alt=\"\" width=\"600\" height=\"224\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-42.jpg 600w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/pasted-image-42-300x112.jpg 300w\" sizes=\"auto, (max-width: 600px) 100vw, 600px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">Initial studies carried out on micro CTs show how<span class=\"Apple-converted-space\">\u00a0 <\/span>XPES can come into contact with most of the canal walls (fig. 7-8), and how it<span class=\"Apple-converted-space\">\u00a0 <\/span>improves the cleansing of all the ovoid section apices.<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17383\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.47.12-48.jpg\" alt=\"\" width=\"589\" height=\"346\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.47.12-48.jpg 589w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.47.12-48-300x176.jpg 300w\" sizes=\"auto, (max-width: 589px) 100vw, 589px\" \/> <img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17384\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.47.35-51.jpg\" alt=\"\" width=\"594\" height=\"344\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.47.35-51.jpg 594w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.47.35-51-300x174.jpg 300w\" sizes=\"auto, (max-width: 594px) 100vw, 594px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">Moreover, considering that it works with a minimum torque (1 Nw), the risks of creating a dentin crack are minimized (video5) [18] and above all, the amount of debris produced by the processing is minimal (fig9).<\/span><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/4_kpM-VzYfo?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17385\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.48.03-57.jpg\" alt=\"\" width=\"518\" height=\"260\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.48.03-57.jpg 518w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Schermata-2017-08-17-alle-18.48.03-57-300x151.jpg 300w\" sizes=\"auto, (max-width: 518px) 100vw, 518px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">As with the XPEF, its austenitic shape is maintained above 35 \u00b0 C, so irrigants must maintain this temperature to have an optimal performance.<\/span><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/0sXITPiizL0?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p class=\"p1\"><span class=\"s1\">From torsional tests, XPES is rather delicate when compared to Flex-Master \u2122 (VDV), Pro-file Vortex Blue \u2122 (Dentply-Tulsa), and True Shape \u2122 (Dentsply-Tulsa) [19] but this is due to the own nature of<span class=\"Apple-converted-space\">\u00a0 <\/span>the file, it does not need a torsional resistance because it does not work engaging in canal walls.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Its swirling movement does not allow the blades to engage in dentin, and this produces<span class=\"Apple-converted-space\">\u00a0 <\/span>less stress on the walls and gives<span class=\"Apple-converted-space\">\u00a0 <\/span>a greater amount of free space inside the canal in which the action of the irrigants can be better performed. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">84% of free space created (compared to 46% of a conventional instrument) allows the debris to be suspended? And and helps to discharge them with the irrigants [21].<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">In the studies, the best results were obtained by continuous irrigation during the milling phase, both with XPEF and with XPES.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The instrument does not want to be a single file and contemplate an initial pre-flaring that has a dual function.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Firstly, the creation of an initial glidepath: especially if performed with rotating instruments [35,43] it minimizes the risk of altering the natural endodontic anatomy by also reducing the stress of the larger instruments.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Secondly, it is possible to reach a diameter of ISO # .15 (minimum 2% conicity) that allows the subsequent introduction of XPES.\u00a0<\/span><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/eo2jTyXv2QE?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p class=\"p1\"><span class=\"s1\">The method involves coming and going movements from 3 to 5 times, trying to reach the apex. If this doesn\u2019t work, the instrument is extracted and cleaned, the canal irrigated, and the same sequence restarts; Once LDL is reached, 10 up-down movements are to be performed along the canal.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">This operation will give the canal the expected conicity and # .30 diameter.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Subsequently you can perform the final cleansing maneuvers to perform with the XPEF [22]\u00a0<\/span><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/AJEpiFHpH4k?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p class=\"p1\"><span class=\"s1\">In 2017, FKG completes the range of these instruments with one dedicated to retreatment : the XP-EndoFinsher-R \u2122 (XPEF-R). The XPEF and XPES capability of removing gutta-percha has already been appreciated previously, with this new instrument with a stronger core than XPEF and a diameter of ISO.30 # in top (Fig10), the quality of cleaning would be superior , although some studies indicate that the difference in gutta clearance between XPEF and XPEF-R is not statistically significant [7].<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17386\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Foto-Rit.001-69.jpg\" alt=\"\" width=\"1280\" height=\"720\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Foto-Rit.001-69.jpg 1280w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Foto-Rit.001-69-300x169.jpg 300w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Foto-Rit.001-69-768x432.jpg 768w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Foto-Rit.001-69-1024x576.jpg 1024w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Foto-Rit.001-69-711x400.jpg 711w\" sizes=\"auto, (max-width: 1280px) 100vw, 1280px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">Once the canal is shaped and cleaned at<span class=\"Apple-converted-space\">\u00a0 <\/span>best , it is possible to proceed with the closing phase using conventional methods, even if, in order to create a three-dimensional seal, this instrumental technique is well conceived both a conceptual and a clinical<span class=\"Apple-converted-space\">\u00a0 <\/span>view by using bioceramics as sealer.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">By doing so, the gutta cone only works as a<span class=\"Apple-converted-space\">\u00a0 <\/span>carrier and a space keeper for any reprocessing, and all the clean space will be occupied by bioceramics. Thanks to their peculiar physical and chemical characteristics, they will adjust<span class=\"Apple-converted-space\">\u00a0 <\/span>to the walls and<span class=\"Apple-converted-space\">\u00a0 <\/span>seal the spaces that the gutta-percha could not reach, even with a vertical hot condensation (fig11)<\/span><\/p>\n<p><iframe loading=\"lazy\" width=\"1170\" height=\"658\" src=\"https:\/\/www.youtube.com\/embed\/8oQZRyXfBcw?feature=oembed\" frameborder=\"0\" allow=\"autoplay; encrypted-media\" allowfullscreen><\/iframe><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17387\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Bioceramic-Sealer.001-67.jpg\" alt=\"\" width=\"1280\" height=\"720\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Bioceramic-Sealer.001-67.jpg 1280w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Bioceramic-Sealer.001-67-300x169.jpg 300w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Bioceramic-Sealer.001-67-768x432.jpg 768w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Bioceramic-Sealer.001-67-1024x576.jpg 1024w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/Bioceramic-Sealer.001-67-711x400.jpg 711w\" sizes=\"auto, (max-width: 1280px) 100vw, 1280px\" \/><\/p>\n<p class=\"p1\"><span class=\"s1\">In addition, this eliminates the problem of the contraction of the material after cooling, which is known to be the cause of gaps on the canal walls, which can fail (Video 9).<\/span><\/p>\n<h2 class=\"p1\"><span class=\"s1\">Gentle File (Medic NRG)<\/span><\/h2>\n<p class=\"p2\"><span class=\"s1\">Compared to the SAF and to the XP-Endo series, the Gentlefile (GF), is the one with the least \u201cexpandable\u201d anatomical<span class=\"Apple-converted-space\">\u00a0 <\/span>characteristics.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">In fact,<span class=\"Apple-converted-space\">\u00a0 <\/span>the instrument does not adapt strictly<span class=\"Apple-converted-space\">\u00a0 <\/span>to the canal<span class=\"Apple-converted-space\">\u00a0 <\/span>by compressing (squeezing) itself, but it<span class=\"Apple-converted-space\">\u00a0 <\/span>exploits the rotation speed and the centrifugal force to reach the canal as close<span class=\"Apple-converted-space\">\u00a0 <\/span>as possible.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">Moreover, the instrument manufacturing <span class=\"Apple-converted-space\">\u00a0 <\/span>bucks the trend, compared to latest instruments.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">As a matter of fact, It is not made from<span class=\"Apple-converted-space\">\u00a0 <\/span>NITI alloy,<span class=\"Apple-converted-space\">\u00a0 <\/span>but from steel and characterized by a series of layers of spiral wrapped wires on a central stem.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">The apical section is more<span class=\"Apple-converted-space\">\u00a0<\/span>flexible and varies depending on the diameters examined.<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-17470\" src=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/FIG12.png\" alt=\"\" width=\"800\" height=\"455\" srcset=\"https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/FIG12.png 800w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/FIG12-300x171.png 300w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/FIG12-768x437.png 768w, https:\/\/www.zerodonto.com\/en\/wp-content\/uploads\/2017\/11\/FIG12-703x400.png 703w\" sizes=\"auto, (max-width: 800px) 100vw, 800px\" \/><\/p>\n<p class=\"p2\"><span class=\"s1\">The surface roughness and the high rotation speed enables a close contact as possible, thus resulting in homogeneous and conservative removal of dentin.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">The procedure implies the use of a set of 6 instruments<span class=\"Apple-converted-space\">\u00a0 <\/span>, to be chosen depending on the type and complexity of the canal treated. method and a universal instrument to perform an endodontic treatment that makes it possible to clean the whole canal, but recent achievements have certainly allowed us to move forward in order to reduce the risks of failure and increase<span class=\"Apple-converted-space\">\u00a0 <\/span>healing percentage.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">We will certainly need<span class=\"Apple-converted-space\">\u00a0 <\/span>further studies to confirm the efficacy of the new alloys and<span class=\"Apple-converted-space\">\u00a0 <\/span>various shaping methods, but the initial clinical experience<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">with the Xp Endo system<\/span><span class=\"s1\">, combined with easiness of use and the results highlighted by<span
