42 years old female patient reached us looking for a corrective treatment for a implant supported prosthetic at the 21 tooth region. The wrong tridimensional position of the implant resulted in a vestibular access to the fixation screw, 10 mm approximately above the gingival margin, causing a great defect at this same region. At first the removal (explanation) of the misplaced implant took place, as well as a connective graft to repair the soft tissue trauma. Secondly was made a guided bone regeneration using a autologous block. A third surgery was necessary for a Bone Level/straumman implant placement (installation).
After the proper osteointegration time, the prosthetic rehabilitation was conducted using a Variobase abutment fitted for CAD/CAM system. A model was sent to straumman for the confection of a zirconium abutment and a further Emax total crow for the definitive prosthetic solution.
The evidence (science) based treatment proposed was very efficient once the patient dissatisfaction have been solved when the expected esthetic was achieved.
